This is made using the Low-Poly Geometrical effect in Photoshop. I've always loved this effect for it's simplicity and kind of whimsical-yet-technological appeal, and so I thought it would be cool to learn how to do it to my own photos. I did pretty much achieve what I'd anticipated, and I'm quite satisfied with the end result (despite having incredibly limited experience in Photoshop and effect work). I challenged myself quite a bit because I'm not so great in stuff like Math and Geometry, so I had to be extra-careful with the shapes i used (to make them all fit together nicely).
